TRT Diary guide
TRT Side Effects Diary: What to Record
A side effects diary helps you describe what happened, when it started, how severe it was, and what else was happening around the same time.
Side effect diary fields
- Symptom name, start date, severity, and whether it is improving or worsening.
- Timing near injections, medication changes, sleep changes, stress, or illness.
- Blood pressure and pulse when relevant.
- Skin changes, headaches, mood changes, fluid retention, sleep changes, or other new symptoms.
- Urgent symptoms and clinician instructions if your doctor has provided them.

TRT side effects diary XLSX
A side-effect log for timing, severity, vitals, possible context, actions taken, and clinician questions. Styled XLSX opens in Excel, Numbers, and Google Sheets. CSV is still available.
Be specific about timing
Side-effect notes are more useful when they include timing. Record the day, severity, what changed recently, and whether the symptom repeated.
If symptoms are severe, sudden, or worrying, contact a healthcare professional or emergency services instead of waiting to update a diary.
Pair symptoms with objective data
When possible, connect side-effect notes to blood pressure readings, lab dates, sleep, illness, and appointment changes. This helps your clinician review patterns more efficiently.
